Bart Van Assche 998cda78d4 block/cgroup: Inline blkg_conf_{open,close}_bdev_frozen()
The blkg_conf_open_bdev_frozen() calling convention is not compatible
with lock context annotations. Fold both blkg_conf_open_bdev_frozen()
and blkg_conf_close_bdev_frozen() into their only caller. This patch
prepares for enabling lock context analysis.

The type of 'memflags' has been changed from unsigned long into unsigned
int to match the type of current->flags. See also <linux/sched.h>.

Split blkg_conf_exit() into blkg_conf_unprep() and blkg_conf_close_bdev()
because blkg_conf_exit() is not compatible with the Clang thread-safety
annotations. Remove blkg_conf_exit(). Rename blkg_conf_exit_frozen() into
blkg_conf_close_bdev_frozen(). Add thread-safety annotations to the new
functions.

Bart Van Assche ea4f575e72 block/cgroup: Split blkg_conf_prep()
Move the blkg_conf_open_bdev() call out of blkg_conf_prep() to make it
possible to add lock context annotations to blkg_conf_prep(). Change an
if-statement in blkg_conf_open_bdev() into a WARN_ON_ONCE() call. Export
blkg_conf_open_bdev() because it is called by the BFQ I/O scheduler and
the BFQ I/O scheduler may be built as a kernel module.

Marco Crivellari 7e712f292e block: Add WQ_PERCPU to alloc_workqueue users
This continues the effort to refactor workqueue APIs, which began with
the introduction of new workqueues and a new alloc_workqueue flag in:

   commit 128ea9f6cc ("workqueue: Add system_percpu_wq and system_dfl_wq")
   commit 930c2ea566 ("workqueue: Add new WQ_PERCPU flag")

The refactoring is going to alter the default behavior of
alloc_workqueue() to be unbound by default.

With the introduction of the WQ_PERCPU flag (equivalent to !WQ_UNBOUND),
any alloc_workqueue() caller that doesn’t explicitly specify WQ_UNBOUND
must now use WQ_PERCPU. For more details see the Link tag below.

In order to keep alloc_workqueue() behavior identical, explicitly request
WQ_PERCPU.

Christoph Hellwig be6e0a533d mm: simplify the mempool_alloc_bulk API
The mempool_alloc_bulk was modelled after the alloc_pages_bulk API,
including some misunderstanding of it.

Remove checking for NULL slots in the array, as alloc_pages_bulk and
kmem_cache_alloc_bulk always fill the array from the beginning and thus
we know the offset of the first failing allocation.  This removes support
for working well with alloc_pages_bulk used to refill page arrays that
might have an entry removed from in the middle, but that is only used by
sunrpc and hopefully on it's way out.

Also remove the allocated parameter as it is redundant because the caller
can simply specific and offset into the entries array.

Yu Kuai 6636e16e60 block, bfq: release cgroup stats with bfq_group
BFQ cgroup stats contain percpu counters embedded in struct bfq_group,
but the old free path destroys them from bfq_pd_free(), which is tied
to blkg policy-data teardown.

That is not the same lifetime as struct bfq_group. BFQ pins bfq_group
while bfq_queue entities refer to it, so bfq_pd_free() can drop the
policy-data reference while other bfq_group references still exist. The
following blkcg change also defers policy-data release through RCU and
leaves BFQ to run the final bfqg_put() from an RCU callback. For that
conversion, stats teardown must belong to the last bfq_group put, not to
policy-data teardown.

Move stats teardown to bfqg_put() so the embedded counters are destroyed
exactly when the last bfq_group reference is released, before kfree(bfqg).

Without this preparatory change, the RCU-delayed policy-data free
conversion reproduced the following KASAN report:

Keith Busch b7f40ab501 block: export passthrough stats enabled
A user can enable io accounting for passthrough requests, so export the
helper that checks if the request should be tracked. This will enable
stacking drivers to to report iostats for passthrough workloads. Since
the stacking request_queue may not be the one providing the request, the
API has to add a parameter for the caller to specify which one to check.

Tao Cui 6235ea3f8b blk-throttle: schedule parent dispatch in tg_flush_bios()
tg_flush_bios() schedules pending_timer on the child tg's own
service_queue, which causes throtl_pending_timer_fn() to dispatch from
the child's pending_tree.  For leaf cgroups this tree is empty, so the
timer fires and exits without dispatching the throttled bio.

The throttled bio sits in the parent's pending_tree with disptime set
to jiffies (THROTL_TG_CANCELING zeroes all dispatch times), but the
parent's timer is never explicitly rescheduled.  The bio only gets
dispatched when the parent timer eventually fires at its previously
scheduled expiry.

Fix by calling throtl_schedule_next_dispatch(sq->parent_sq, true)
instead, matching what tg_set_limit() already does.  This forces the
parent's dispatch cycle to run immediately and flush all canceling
bios without waiting for a stale timer.

For the device deletion path (blk_throtl_cancel_bios), directly
complete throttled bios with EIO via bio_io_error() instead of
dispatching them through the timer -> work -> submission chain.
This avoids a race with the SCSI state machine where bios can reach
the SCSI layer while the device is in SDEV_CANCEL state, causing
ENODEV instead of the expected EIO.

Mike Rapoport (Microsoft) 17d7492a50 block: partitions: replace __get_free_page() with kmalloc()
check_partition() allocates a buffer to use as backing memory for
seq_buf.

This buffer can be allocated with kmalloc() as there's nothing special
about it to go directly to the page allocator.

kmalloc() provides a better API that does not require ugly casts and
kfree() does not need to know the size of the freed object.

For a single allocation on the cold path the performance difference between
kmalloc() and __get_free_pages() is not measurable as both allocators take
an object/page from a per-CPU list for fast path allocations.

For the slow path the performance is anyway determined by the amount of
reclaim involved rather than by what allocator is used.

Replace use of __get_free_page() with kmalloc() and free_page() with
kfree().

Chaitanya Kulkarni 7882834048 block: clear BLK_FEAT_PCI_P2PDMA in blk_stack_limits() for non-supporting devices
BLK_FEAT_NOWAIT and BLK_FEAT_POLL are cleared in blk_stack_limits()
when an underlying device does not support them.  Apply the same
treatment to BLK_FEAT_PCI_P2PDMA: stacking drivers set it
unconditionally and rely on the core to clear it whenever a
non-supporting member device is stacked.

Keith Busch b051bb6bf0 blk-mq: reinsert cached request to the list
A previous commit removed an optimization out of caution for a scenario
that turns out not to be real: all the "queue_exit" goto's are safe to
reinsert the request into the cached_rq's plug list as they are either
from a non-blocking path, or a successful merge that already holds the
queue reference. This optimization is most needed for small sequential
workloads that successfully merge into larger requests.

Tal Zussman ee9895ae5a block: remove blkdev_write_begin() and blkdev_write_end()
Remove blkdev_write_begin(), blkdev_write_end(), and their entries in
def_blk_aops. These have been unreachable since commit 487c607df7
("block: use iomap for writes to block devices") switched block device
buffered writes from generic_perform_write() to
iomap_file_buffered_write(), which bypasses aops->write_begin/end.

Denis Arefev f73aa66dff block: Avoid mounting the bdev pseudo-filesystem in userspace
The bdev pseudo-filesystem is an internal kernel filesystem with which
userspace should not interfere. Unregister it so that userspace cannot
even attempt to mount it.

This fixes a bug [1] that occurs when attempting to access files,
because the system call move_mount() uses pointers declared in the
inode_operations structure, which for the bdev pseudo-filesystem
are always equal to 0. `inode->i_op = &empty_iops;`

Mateusz Nowicki b040a1a452 block: switch numa_node to int in blk_mq_hw_ctx and init_request
numa_node in blk_mq_hw_ctx and the matching argument of
blk_mq_ops::init_request can be NUMA_NO_NODE (-1).  Declared as
unsigned int, NUMA_NO_NODE becomes UINT_MAX and walks off
nvme_dev::descriptor_pools[] on CONFIG_NUMA=n [1].

Switch the field and the callback prototype to int and update all
in-tree init_request implementations.  No functional change:
cpu_to_node(), kmalloc_node() and blk_alloc_flush_queue() already
take int.

Chao Shi 49f06cff50 block: skip sync_blockdev() on surprise removal in bdev_mark_dead()
bdev_mark_dead()'s @surprise == true means the device is already gone.
The filesystem callback fs_bdev_mark_dead() honours this and skips
sync_filesystem(), but the bare block device path (no ->mark_dead op)
lost its !surprise guard when the holder ->mark_dead callback was wired
up (see Fixes), and now calls sync_blockdev() unconditionally, which can
hang forever waiting on writeback that can no longer complete.

syzkaller hit this via nvme_reset_work()'s "I/O queues lost" path:
nvme_mark_namespaces_dead() -> blk_mark_disk_dead() ->
bdev_mark_dead(bdev, true) -> sync_blockdev() blocks in
folio_wait_writeback(), wedging the reset worker and every task waiting
on it.

Skip the sync on surprise removal, matching fs_bdev_mark_dead();
invalidate_bdev() still runs. Orderly removal (surprise == false) is
unchanged.

Found by FuzzNvme(Syzkaller with FEMU fuzzing framework).

Aaron Tomlin 4d94ec1bc1 blk-mq: add tracepoint block_rq_tag_wait
In high-performance storage environments, particularly when utilising
RAID controllers with shared tag sets (BLK_MQ_F_TAG_HCTX_SHARED), severe
latency spikes can occur when fast devices (SSDs) are starved of hardware
tags when sharing the same blk_mq_tag_set.

Currently, diagnosing this specific hardware queue contention is
difficult. When a CPU thread exhausts the tag pool, blk_mq_get_tag()
forces the current thread to block uninterruptible via io_schedule().
While this can be inferred via sched:sched_switch or dynamically
traced by attaching a kprobe to blk_mq_mark_tag_wait(), there is no
dedicated, out-of-the-box observability for this event.

This patch introduces the block_rq_tag_wait tracepoint in the tag
allocation slow-path. It triggers immediately before the task state
is altered to TASK_UNINTERRUPTIBLE (ensuring safety for PREEMPT_RT
locks). It exposes the exact hardware context (hctx) that is starved,
the specific pool experiencing starvation (driver, software scheduler,
or reserved), and the exact pool depth.

This provides storage engineers with a zero-configuration, low-overhead
mechanism to definitively identify shared-tag bottlenecks. For example,
userspace can trivially replicate tag starvation counters using bpftrace:

Wentao Liang 148cd48731 block: partitions: fix of_node refcount leak in of_partition()
of_partition() calls of_node_get() on the parent device node at the
beginning of the function, storing the reference in 'partitions_np'.
This reference is leaked in two paths:

1. The compatibility check at the top of the function returns 0
   without releasing partitions_np when the node exists but is not
   "fixed-partitions" compatible.

2. The function returns 1 at the end after successfully processing
   all partitions without releasing partitions_np.

Fix both leaks by adding of_node_put(partitions_np) on each path.

Damien Le Moal f698276991 block: avoid use-after-free in disk_free_zone_resources()
The function disk_update_zone_resources() may call
disk_free_zone_resources() in case of error, and following this,
blk_revalidate_disk_zones() will again calls disk_free_zone_resources() if
disk_update_zone_resources() failed. If a zone worker thread is being used
(which is the default for a rotational media zoned device),
disk_free_zone_resources() will try to stop the zone worker thread twice
because disk->zone_wplugs_worker is not reset to NULL when the worker
thread is stopped the first time.

In disk_free_zone_resources(), fix this by correctly clearing
disk->zone_wplugs_worker to NULL when the worker thread is stopped.

And while at it, since disk_free_zone_resources() is always called after a
failed call to disk_update_zone_resources(), remove the unnecessary call
to disk_free_zone_resources() in disk_update_zone_resources().

Keith Busch dc278e9bf2 blk-mq: pop cached request if it is usable
When submitting a bio to blk-mq, if the task should sleep after peeking
a cached request, but before it pops it, the plug flushes and calls
blk_mq_free_plug_rqs, freeing the cached_rqs. This creates a
use-after-free bug. Fix this by popping the cached request before any
possible blocking calls if it is suitable for use.

Popping this request first holds a queue reference, so avoid any
serialization races with queue freezes and can safely proceed with
dispatching that request to the driver. This potentially increases a
timing window from when a driver wants to freeze its queue to when
requests stop being dispatched. That scenario is off the fast path
though, and drivers need to appropriately handle requests during a
freeze request anyway.

The downside is the popped element needs to be individually freed when
we performed a bio plug merge. The cached request would have had to be
freed later anyway, but this patch does it inline with building the plug
list instead of after flushing it.

Qing Ming 8817005efb cgroup/rstat: validate cpu before css_rstat_cpu() access
css_rstat_updated() is exposed as a BPF kfunc and accepts a
caller-provided cpu argument. The function uses cpu for per-cpu rstat
lookups without checking whether it refers to a valid possible CPU.

A BPF iter/cgroup program with CAP_BPF and CAP_PERFMON can pass an
invalid cpu value. On an unfixed UBSCAN_BOUNDS test kernel, cpu ==
0x7fffffff triggers:

  UBSAN: array-index-out-of-bounds in kernel/cgroup/rstat.c:31:9
  index 2147483647 is out of range for type 'long unsigned int [64]'

Damien Le Moal 836efd35c4 block: fix handling of dead zone write plugs
Shin'ichiro reported hard to reproduce unaligned write errors with zoned
block devices. Under normal operation conditions (e.g. running XFS on an
SMR disk), these errors are nearly impossible to trigger. But using a
"slow" kernel with many debug options enables and some specific use
cases (e.g. fio zbd test case 46), the errors can be reproduced fairly
easily.

The unaligned write errors come from mishandling a valid reference
counting pattern of zone write plugs. Such pattern triggers for instance
if a process A writes a zone (not necessarilly to the full state),
another process B immediately resets the zone and immediately following
the completion of the zone reset, starts issuing writes to the zone.
With such pattern, in some cases, the zone write plugs worker thread of
the device may still be holding a reference to the zone write plug of
the zone taken when process A was writing to the zone. The following
zone reset from process B marks the zone as dead but does not remove the
zone write plug from the device hash table as a reference to the plug
still exist. Once process B starts issuing new writes, the zone write
plug is seen as dead and the writes from process B are immediately
failed, despite this write pattern being perfectly legal.

Fix this by allowing restoring a dead zone write plug to a live state if
a write is issued to the zone when the zone is: marked as dead, empty
and the write sector corresponds to the first sector of the zone (that
is, the write is aligned to the zone write pointer). This is done with
the new helper function disk_check_zone_wplug_dead(), which restores a
dead zone write plug to a live state by clearing the BLK_ZONE_WPLUG_DEAD
flag and restoring the initial reference to the zone write plug taken
when the plug was added to the device hash table.

pin_user_pages_fast() can partially succeed and return the number of
pages that were actually pinned. However, the bio_integrity_map_user()
does not handle this partial pinning. This leads to a general protection
fault since bvec_from_pages() dereferences an unpinned page address,
which is 0.

To fix this, add a check to verify that all requested memory is pinned.
If partial pinning occurs, unpin the memory and return -EFAULT.

Casey Chen 2c6e6a18a3 block: recompute nr_integrity_segments in blk_insert_cloned_request
blk_insert_cloned_request() already recomputes nr_phys_segments
against the bottom queue, because "the queue settings related to
segment counting may differ from the original queue." The exact same
reasoning applies to integrity segments: a stacked driver's underlying
queue can have tighter virt_boundary_mask, seg_boundary_mask, or
max_segment_size than the top queue, in which case
blk_rq_count_integrity_sg() against the bottom queue produces a
different count than the cached rq->nr_integrity_segments inherited
from the source request by blk_rq_prep_clone().

When the cached count is lower than the bottom queue's actual count,
blk_rq_map_integrity_sg() trips

	BUG_ON(segments > rq->nr_integrity_segments);

on dispatch. The same families of stacked setups that motivated the
existing nr_phys_segments recompute -- dm-multipath fanning out to
nvme-rdma in particular -- can produce this.

Mirror the nr_phys_segments handling: when the request carries
integrity, recompute nr_integrity_segments against the bottom queue
and reject the request if it exceeds the bottom queue's
max_integrity_segments. blk_rq_count_integrity_sg() and
queue_max_integrity_segments() are both already available via
<linux/blk-integrity.h>, which blk-mq.c includes.

This closes a latent gap in the stacking contract and brings the
integrity-segment accounting in line with the existing
phys-segment accounting.

David Carlier 637ad3a56a block: don't overwrite bip_vcnt in bio_integrity_copy_user()
bio_integrity_add_page() already sets bip_vcnt to 1 for the bounce
segment. Overwriting it with nr_vecs breaks bip_vcnt <= bip_max_vcnt
on WRITE (bip_max_vcnt is 1), so the gap-merge checks in block/blk.h
read past the bip_vec[] flex array. On READ the read is in bounds
but lands on a saved user bvec instead of the bounce.

The line was added for split propagation, but bio_integrity_clone()
doesn't copy bip_vcnt and BIP_CLONE_FLAGS excludes BIP_COPY_USER.

Eric Biggers 623463c8db block: export blk-crypto symbols required by dm-inlinecrypt
bio_crypt_set_ctx(), blk_crypto_init_key(), and
blk_crypto_start_using_key() are needed to use inline encryption; see
Documentation/block/inline-encryption.rst.  Export them so that
dm-inlinecrypt can use them.  The only reason these weren't exported
before was that inline encryption was previously used only by fs/crypto/
which is built-in code.

Jens Axboe 212ec34e4e block: only read from sqe on initial invocation of blkdev_uring_cmd()
This passthrough helper currently only supports discards. Part of that
command is the start and length, which is read from the SQE. It does
so on every invocation, where it really should just make it stable
on the first invocation. This avoids needing to copy the SQE upfront,
as we only really need those two 8b values stored in our per-req
payload.

Naman Jain 41c665aae2 block: relax pgmap check in bio_add_page for compatible zone device pages
bio_add_page() and bio_integrity_add_page() reject pages from different
dev_pagemaps entirely, returning 0 even when those pages have compatible
DMA mapping requirements. This forces callers to start a new bio when
buffers span pgmap boundaries, even though the pages could safely coexist
as separate bvec entries.

This matters for guests where memory is registered through
devm_memremap_pages() with MEMORY_DEVICE_GENERIC in multiple calls,
creating separate dev_pagemaps for each chunk. When a direct I/O buffer
spans two such chunks, bio_add_page() rejects the second page, forcing an
unnecessary bio split or I/O failure.

Introduce zone_device_pages_compatible() in blk.h to check whether two
pages can coexist in the same bio as separate bvec entries. The block DMA
iterator (blk_dma_map_iter_start) caches the P2PDMA mapping state from the
first segment and applies it to all others, so P2PDMA pages from different
pgmaps must not be mixed, and neither must P2PDMA and non-P2PDMA pages.
All other combinations (MEMORY_DEVICE_GENERIC pages from different pgmaps,
or MEMORY_DEVICE_GENERIC with normal RAM) use the same dma_map_phys path
and are safe.

Replace the blanket zone_device_pages_have_same_pgmap() rejection with
zone_device_pages_compatible(), while keeping
zone_device_pages_have_same_pgmap() as a merge guard.
Pages from different pgmaps can be added as separate bvec entries but
must not be coalesced into the same segment, as that would make
it impossible to recover the correct pgmap via page_pgmap().

Naman Jain 13920e4b7b block: add pgmap check to biovec_phys_mergeable
biovec_phys_mergeable() is used by the request merge, DMA mapping,
and integrity merge paths to decide if two physically contiguous
bvec segments can be coalesced into one. It currently has no check
for whether the segments belong to different dev_pagemaps.

When zone device memory is registered in multiple chunks, each chunk
gets its own dev_pagemap. A single bio can legitimately contain
bvecs from different pgmaps -- iov_iter_extract_bvecs() breaks at
pgmap boundaries but the outer loop in bio_iov_iter_get_pages()
continues filling the same bio. If such bvecs are physically
contiguous, biovec_phys_mergeable() will coalesce them, making it
impossible to recover the correct pgmap for the merged segment
via page_pgmap().

Add a zone_device_pages_have_same_pgmap() check to prevent merging
bvec segments that span different pgmaps.
